Alan Patrick put the hosts ahead from the penalty spot before Carlos Vinicius equalized with a glancing header.
Patrick converted another spot-kick but the visitors again responded through Andre Henrique, who nodded in from a corner on the stroke of halftime.
Alysson, a 62nd-minute replacement for Andre Henrique, scored moments after his introduction by completing a swift counterattack with a clinical finish past goalkeeper Sergio Rochet.
Both sides finished the match with 10 men after late red cards to Gremio's Arthur and Internacional's Alexandro Bernabei.
The result at Beira-Rio stadium leaves Gremio 12th in the 20-team standings with 28 points, one ahead of 13th-placed Internacional.
