Operating mistake caused gas leak in fatal poisoning on cargo ship

An operating mistake by a cargo ship crew member caused fatal poisoning onboard the ship in east China, authorities said Thursday.

Ten people died and 19 were hospitalized after carbon dioxide leaked in the cargo ship at Longyan Port, Weihai city, in Shandong province last week.

Zhang Pengfei, one of the chief investigators, said the cargo ship crew member was writing down parameters in a room where carbon dioxide tanks were located. The staff accidentally touched and turned on the valve of a tank, causing carbon dioxide to leak into the ship's system.

While trying to contain the leak, the crew member mistakenly turned on the pressure-increasing valve, which caused gas to push open other valves of the carbon dioxide tanks and the main valve between the room and the cabin. A large amount of carbon dioxide leaked into the cabin, poisoning crew members that were repairing the ship.

The carbon dioxide was part of the ship's fire control system, Zhang said.
