Inter defender Andrea Ranocchia headed in the decisive goal seconds before the end of the extra time to move the home side past Bologna.
Fredy Guarin put Inter 1-0 ahead in the 34th minute, and Rodrigo Palacio doubled their lead in the 78th minute.
But Bologna refused to gave in and battled back to regain parity through Alessandro Diamanti and Alberto Gilardino in the dying minutes of the second half.
As the penalty shoot-out seemed inevitable, Ranocchia headed in the winner from a corner.
In the semifinals, Inter will meet either Roma or Fiorentina. Juventus and Lazio will play against each other in the other semifinal.
